Requesting a Chosen Name for Students and Employees

OCC recognizes that students and employees may prefer to use first names other than their legal first name to identify themselves.
Students and employees are not required to obtain a court-ordered name change to change their OCC records as a prerequisite to being addressed by their chosen name.
Students
Complete ADS 111, Chosen First Name and Personal Pronoun Request, To verify the identity of the requestor, a valid OCC ID or government ID will be
required with this form in order for changes to be processed unless it is sent from
your OCC student email account. Submit the completed form and proof of ID in person
at any Enrollment Services Office on any campus, via USPS, or email to the address
printed on the form.
Employees
Complete HR-30, Employee Change of Information Request form. Direct any questions to hr@oaklandcc.edu.
